How it works
At low doses, TCAs modulate visceral hypersensitivity via central pain processing and slow gut transit, benefiting IBS-D. Mechanism is distinct from their antidepressant action.
Requires a prescription. Constipating effect makes this more appropriate for IBS-D than IBS-C. Side effects include dry mouth and sedation. Do not stop abruptly.
A Cochrane review of antidepressants for IBS found significant benefit, with NNT around 4. TCAs have more supporting data for IBS-D than SSRIs.
