How it works

Aspirin inhibits prostaglandin synthesis and platelet aggregation. Metoclopramide enhances aspirin absorption (impaired during migraine) and reduces nausea.

Metoclopramide requires a prescription in many countries and can rarely cause tardive dyskinesia with prolonged use. Aspirin is contraindicated in children under 16 and with anticoagulants.

Why strong evidence

Multiple RCTs including Cochrane reviews show this combination provides relief comparable to triptans for many patients and is a cost-effective first-line option.
