How it works

Increase synaptic serotonin (SSRIs) or serotonin and norepinephrine (SNRIs). Reduce amygdala hyperreactivity and improve prefrontal regulation of fear responses over weeks.

Require a prescription. Initial worsening of anxiety possible in weeks 1–2 β€” start at half dose. Do not stop abruptly. Sexual side effects are common. May take 6–8 weeks for full benefit.

Why very strong evidence

Meta-analyses confirm SSRIs and SNRIs are highly effective for generalised anxiety, panic disorder, and social anxiety. Combination with CBT outperforms either alone.
